Request an Appointment

You can request an appointment with a Doctor or Nurse Practitioner by clicking this Patchs link. Once you have logged into your Patchs account, select the “New health problem” OR “Ongoing health problem” form option. Your request will be assessed and assigned to the best person to help you with your concern.

Please ensure you check your emails regularly for our response.


Routine Appointments

Routine appointments are usually booked or arranged in advance. You can request an appointment with a Practice Nurse or Health Care Assistant by clicking this Patchs link. Once you have logged into your Patchs account, select “ADMIN” from the list of options and include the reason for your request to ensure we allocate the correct amount of time.

    • Blood tests (These are carried out at our Caxton Villa site)

    Home Visits

    Home visits will be arranged with prior agreement from our team of clinical practitioners.

    Home visits are for housebound (physically unable to attend the practice), palliative and some of our more vulnerable patients.

    Out of Hours

    Out of hours (commissioned by the ICB) can be contacted by calling 111 or visiting https://111.nhs.uk/ for medical advice after 18:30 and before 08:00 or at weekends.

    • view parts of your GP record – including information about medication, allergies, vaccinations, previous illnesses and test results.
